A home care shift schedule shows who has accepted responsibility during each agreed period, when handover occurs and which gaps remain uncovered. Include family cover, paid shifts, breaks and relief in the same view. A live-in worker’s presence is not continuous active duty; nursing requirements and appropriate staffing must be confirmed separately with qualified professionals and the provider.

Start with the real week

Choose one calendar week and enter actual dates. Include evenings when relatives work late, weekends and planned absence. Mark a person as confirmed only after they agree. Being in a family group or usually living nearby does not make someone the default backup for an uncovered period.

Keep an open-actions box below: gap, owner, proposed solution and confirmation deadline. Do not delete a gap until a suitable person and scope have been confirmed. If clinical cover is required, a family name or attendant shift should not be substituted without appropriate professional guidance.

Check the joins between shifts

Compare one finish time with the next start time. Is overlap needed, and is it included in the quote? Who provides access to the incoming worker? What happens if the next person is delayed? Record those answers next to the relevant transition instead of relying on a general promise that someone will manage.

For overnight duty, state awake or the specifically agreed sleeping arrangement. For live-in work, mark rest and days off rather than filling every row with the same name. A schedule should make workload understandable to both the household and the worker.

Update without creating competing versions

Put a version date and coordinating contact at the top. When a shift changes, record who requested it, who accepted it and which related handover must change. Notify only the people who need the update and keep the current version in one agreed place.

At the end of the week, compare planned and delivered cover using factual notes. Repeated gaps, extensions or interrupted rest suggest that the staffing proposal needs revision. Ask the provider to review scope and price, and seek qualified advice if the person’s care needs have changed. Do not treat the roster itself as a clinical care plan.
